still having trouble

patient was in a SNF from April to present...original problem bowel obstruction/hernia repair....required prolonged rehab due to weakness and limited mobility....pt is morbidly obese...cellulitis and gout treatment completed more than 14 days prior to home care admit....med changes also made more than 14 days for afib and depression...(M1011 and M1017) .other co-mobordiities...anxiety depression hx uti afib ..chf...incontinence (mixed)...need help with primary DX and sequencing ths one since it was such an extensive rehab stay any help will be greatly appreciated

    The clinician's documentation should determine what should be the primary diagnosis. What skill is being provided and related to what diagnosis. Hope that helps :)
