I know this has been asked before but I just want to clarify. If the patient has htn and chf we now assume a relationship correct? The same as we assume a relationship between htn and ckd without provider documentation?

The new guidance regarding the term “with” makes it acceptable now

When we get the documentation from the physician I find that they are often
listed separately; ie HTN
CHF
My understanding is that they physician has to state HTN with CHF.
Am I interpreting this correctly?

Yes, they are often listed separately when I see them too. I'm most confused about if they can be listed separately or if they have to say "with."
