Patient discharged from hospital with primary dx of "complicated loculated left pleural effusion" patient with dx of bacterial pneumonia and chronic systolic and diastolic heart failure as well. My note from the physician (f2f) Pleural Effusion NEC. Should I code to Pleural Effusion J90 or J91?

    If you have the documentation for the more specific, you should code to highest specificity.
