Patient also had a lucunar stroke which I need more info on..

There is no note excluding Lewy body dementia from Alzheimer's so I would think you can code both based on that documentation. I would wait though until you get further clarification of the stroke because the vascular pathology comment makes me wonder if vascular dementia should be coded.
