pt has afib(unspecified).....new bradycardia...
Diabetes...ckd stage 3...HTN...ac on chr chf...the new guideline we can combine DM & CKD? CKD & HTN? please help

  • You can't assume the CHF with the HTN so it wouldn't be I13.XX

    Only the HTN with the CKD and the CKD with the DM.

    It would be I12.9

    Just to clarify. Does the physician have to state HTN with CHF and CKD or can the summary just state Pt with a history of CHF, COPD, HTN, osteoporosis, and CKD and we can assume the relationship?

    So in the2016 Diagnosis Coding Manual on page 247 under Hypertension with heart involvement it states (conditions in I51.4-I51.9 due to Hypertension) see Hypertension heart. Is that a typo or am I not looking in the right spot to get to the CHF's being included?
