Coding moisture related skin breakdown
Needing advice on what to code for "moisture related skin breakdown". Nurse found what she initially called "Incontinence wound" on patients hip. When I asked for MD confirmed diagnosis we received "okay to use calmoseptine on moisture related skin breakdown". This may be description WOCN using to describe such an area of breakdown but I do not know "type" of wound to code. Any advice is appreciated.

The constant moisture irritates the skin like a diaper rash which is also a dermatitis.
