Dementia/Mild cognitive impairment

Ok, I see this a lot and just want to ask how others code a patient who has documented with Dementia and Mild cognitive impairment. Do you only code the Dementia or do you code both Dementia And the Impairment? Right now I have a patient that has vascular dementia and also noted to have mild cognitive impairment.
