effective january 1, 2017, home health cannot list a stage 3 or 4 pressure ulcer that has fully epithelialized in the OASIS questions because it is now considered "healed" However the site has a reduction in tensile strength and should be observed for poterntial breakdown and should have preventative measures. You can matk "yes" in M1350 if you are observing, monitoring and applying preventative interventions. It is up to clinician/coder if you want to list the diagnosis, but if you decide to do so, it would be lower on the list, probably below the top 6 diagnoses.

My understanding regarding healed pressure ulcers is that you still do not reverse stage. You can comment in narrative about pressure injuries of Stage 3 and 4 in narrative if they are healed or no longer "open", but that you DO NOT code nor put in as an ulcer receiving care in the integument section of Oasis. You would probably still want orders for teaching on pressure ulcer prevention and monitoring of skin since patient is predisposed to pressure ulcer development.
