Is it ok to code off the F2f? Er note states LLE leg swelling and the F2f indicates patient has Venous insufficiency LLE edema?

The FTF encounter is no longer a form but is the actual physician's documentation of time spent with the patient discussing the need for HH and why.
That said, the physician-signed document stating a medical condition is what you code; the doctor being the one who certifies your patient for HH, and has documented an actual encounter (FTF) with the patient in which the condition is documented as well justifying HH.
That said, the Venous Insufficiency is a diagnosis. LE swelling is a sign/symptom.
