I need help coding this: Documentation states the following "SIRS, evidence of sepsis on presentation with tachypnea, tachycardia and elevated WBC." Then it states "Acute on Chronic Kidney disease...possibly likely due to the infection." and "UTI" and it does state the organism is Klebsiella Pneumonia for the UTI.
So, there is Severe Sepsis - R65.20
SIRS without organ involvement R65.10
or SIRS with organ involvement R65.11
Which one do I use?
