Hypertensive heart disease and has Chronic Diastolic Hf and noted chronic ischemic HD?

Can someone please help me code this? Hypertensive heart disease and has Chronic Diastolic Hf it brought me to I11.0, I50.32. Do I not code the chronic ischemic HD? FOC is for the Aortic insuff. And CABG?
