pt fell and fracture left humerus in march,2016. had complications with hardware. went back in hospital and had removal of hardware with revision in may, 2016. how would I code this fx?

    Please do adjustment 16 then 10 and then 9 to remove the charges after 3 31 from this payor and let me know when you are done.
