Sequencing/ Joint Replacement
1. If the primary diagnosis is aftercare for joint replacement does the presence of the artificial joint have to be sequenced directly under it or can be sequenced towards the bottom.
2. Also, In hospice, If someone's terminal DX is lung cancer and they had a personal history of smoking, do they need too be sequenced together? Another coder was under the impression they had to be together but I cannot find any documentation of this.
