Denials because of corresponding OASIS issue

CMS confirmed to me that there have been occasions where a claim will be denied because of failure to find an OASIS assessment due to a mismatch of beneficiary information, such as their Medicare number, on the claim and assessment.

Has anyone experienced this? I'm working on a story and I'd love to hear from you.
