Anxiety and Depression

If a physician documents in an H&P listing of diagnosis Anxiety and depression listed separately should these be coded separately, at what point would I code them as Anxiety Depression F41.8? Does it need to be documented as Anxiety "with" depression or Depression "with" Anxiety before I can code it as such? What about if it says Anxiety "and" Depression not "with"?
I am confused on this one when to list them separately versus as the combined code.
Any help would be greatly appreciated.

    I only code them together when the physician has done so as well. If they
    are listed separately then I code them separately. Would be nice to know
    what others do as well :)
