ASHD with Hypertension

Would I code ASHD with Hypertension as I11.9 or code them separately as I25.10 and I10? Documented by physician as ASHD with Hypertension.

    I would code them separately as ASHD doesn't fall within the conditions classified in I50.-- and I51.4-I51.9 per the includes note and the special coding instructions at the front of the chapter.
