Dementia as Primary dx

Should I code Dementia as primary? It is the reason for the patients physical decline. The physician documented it as the reason for homecare.

What are you thoughts on this? Do you code it?

    If the primary focus of care is due to dementia, and there is supporting documentation and treatment planning reflecting that then yes, I would code it as primary.
