fem/pop complications, diabetes, PVD and arterial ulcers

pt with documented diabetes, PVD. had fem/pop bypass on 021817, developed post op complications of thrombosed bypassed graft. had revision surgery on 022017. now home with surgical wound/incision and arterial ulcers ( per md doc) to right foot. so I have completely confused myself, I need help with this one please:

I am thinking coding would be: complication code for fem/pop T82..
Diabetes with PVD ( due to the with rule)
diabetes with foot ulcer ( due to with rule)
followed by ulcer codes for foot ulcer.

  • It would be the complication of the thrombosed graft with the D if they removed the thrombus. The ulcers are documented as arterial which is a stated cause so you do presume the relationship between the PVD and DM to code E11.51 but you simply follow that with the appropriate L97 code. You would not presume the ulcers are diabetic since the documentation states they are arterial.
