diagnosis question

I have a friend that is a coder for a home health agency and her staff is trying to make her code from nursing home sheets that just have a list of diagnosis codes. She knows that she cannot do this but she is getting a lot of push back because she won't. I have tried to find written information for her to take to them to back her decision up. Does anyone know where I could get this information for her?? Any help will be much appreciated.

  • You could start by asking if those diagnosis are signed by the MD. Usually they are not.
  • Hi Brenda.
    When a patient is referred to a skilled nursing facility from an acute care facility such as an acute care hospital or an LTACH, then the SNF will have copies of the Medicals from that facility which should have included a medical H&P and a DC summary. This could be a starting point. Also, the facesheet and admission information as well as the H&P from the admitting SNF will be helpful. Most SNF's want to send nurses notes and a physician's order for the HH at dc and I find this to be inadequate. Your HH consent also allows you to gather information from the referring facility (to the SNF) that you can request on your own for your HH admission.

  • In the front of the ICD-10 Coding Book under Official Guidelines for Coding and Reporting - The physician or qualified health care practitioner who is legally accountable for establishing the patient's diagnosis. Oasis assessment strategies allow diagnosis information to be obtained through assessment WITH verification from the physician. If the sheets are not signed by the MD, you could query the MD for verification of those diagnosis.
