Hospice Terminal Coding

Our facility is having an issue with hospice diagnosis coding and following the rules of "with" IE a patients terminal diagnosis is CHF but they also have HTN so that has to be primary but it does not match the 485. They are requesting that we code the CHF primary with HTN as secondary. I believe this is incorrect does anyone have a similar issue and can you give me insight if that is following hospice compliance guidelines?

    All hospice and home health coding must comply with the current official coding guidelines and regulations. This statement is repeated in the CMS Chapter 11 Hospice Claims Processing Manual.

    When I come across a similar scenario, I enter a note in the record stating that sequencing of hypertension and CHF was done in accordance with current Coding Clinic guidance.
