Please advise how to code these Dx - HTN w/ chronic renal atrophy and CHF.

    Hmmm. This one is a bit tricky. Here is my 2 cents.

    First, look to see if the hypertension can be linked to the renal atrophy (it can via coding tip at N26.1 - renal atrophy - which suggests I12.9 followed by N26.1).

    Second, look to see if the hypertension can be linked to the CHF. That depends, is there also a verified diagnosis somewhere in the medical record that falls within 151.4 - 151.9? If so, you may link the hypertension to the CHF (which means that you may now also link hypertension with CKD and CHF via I13.0).

    This will look like: I13.0, I50.-, N26.1.

    If you cannot link the hypertension to the CHF, then you'll keep I12.9 followed by N26.1 then code your heart failure I50.-.
