Lower leg ulcers on Recertification

Hi, somewhere I think I've seen (and can't find now)...that you shouldn't change the severity level of a leg ulcer on recert but leave as original until healed. Is this correct?
Say on soc it was Fat layer exposed but now on recert, it appears as skin level.
Can someone let me know where to find the guidance that supports it if you aren't to change it?

    no, the codes should always be updated to the patient's current condition at the time of re certification... of the wound improved, that needs to be reported.
