diabetic pressure ulcer

Documentation states patient has a stage 3 diabetic pressure ulcer left foot. If I code the E11.621 there is a note that says I need to add an L97 code, but the pressure ulcer is l89 code. has anyone else had this issue and what should I assign?

  • Pressure ulcers are due to pressure not diabetes. The healing would be complicated by the diabetes. If MD documents it as a pressure ulcer code the L89.

Diabetic ulcers are not staged, only pressure ulcers are staged. Also as
Jamie said diabetes does not cause pressure ulcers, You can query the
physician to decide if the ulcer is a pressure ulcer or a diabetic ulcer.
It cannot be both, although it can be a pressure ulcer with diabetes as a
co-morbidity that will impact the healing.
