reading about this item, is this item to remain blank if the diagnosis of DM, PAD, PVD does not exist at the SOC on the OASIS C2

  • No the M1028 is code specific and does not address combinations codes. If your patient has PVD/PAD and Diabetes then you would only be coding the E11.51. Therefore you would only mark Diabetes in M1028. The only time you will mark both is when you are using a DM code and an I70 or I73.9 code as well.
