Pressure Ulcers

I saw some guidance awhile back regarding pressure ulcers that was supposed to be in effect as of January 1. I vaguely remember something about reverse staging of a stage 3 or 4, do we still do that? Do we list a healed pressure injury even though it is healed and we are watching it? Can anyone clarify?
