Hospice coding-DM type 2 w/ CKD

Patient Primary Dx is Hypertensive heart failure w/CKD stage 3, chronic diastolic HF, CKD stage 3, DM type 2. Since we have been instructed to code DM w/ CKD, how would I code this if MD is stating that the DM 2 is unrelated?
