Confusion related to UTI

Guidelines say to code the cause of a mental status change vs. using a symptom code for confusion, and I get that.

But do we continue to code a UTI in M1021/M1023 when it's documented as resolved and there is no active treatment?

I'm leaning toward coding the mental status change and NOT the UTI but I'd add Z87.440 - History of UTI.

Does sound right to you?

    No! you don't code the UTI in M1023 and the altered mental should have resolved. You can use personal history of UTI if the PT is at risk of developing recurrent UTI.

  • Thank you. I wasn't as clear as I should have been and mentioned that the altered mental status was not resolved at the time of admission to home care. It normally should have resolved by then but had not. They did a CT, MRI, and intracranial vascular studies etc., and never came up with a definitive diagnosis to explain it so it retained the altered mental status diagnosis. It has been very slowly improving and she is nearly back to base line now... and she does not want another UTI ever again!
