Is the type of dementia mentioned? i.e. Alzheimer's, senile degeneration of brain, Parkinson's. If not then a terminal diagnosis would need to be given and documented by the physician. Once the type of dementia is identified then you would code the terminal diagnosis as primary and follow with the type of dementia. example; G31.0 Alzheimer's with early onset, F02.8- dementia with/without behavioral disturbance and then follow with the other conditions the patient has. If coding weight loss R63.4, also code the BMI Z68 for adults 20 and older.
