hospice primary

I'm not sure how to sequence this. MD documented CKD stage 4 for primary hospice dx. With related conditions of htn, chf, diabetes. For Homecare I would sequence this i13.0, i50.9, E11.22, N18.4. Do I leave the primary N18.4 like she stated??

    Follow current coding guidelines. That is a requirement under CMS the Conditions of Participation.

    OK, so we would sequence it as we would for homecare?

    Yes. Coding must always follow current guidelines. You can enter a coding note that sequencing was done in accordance with ICD-10-CM guidelines.
