It is on attachment A. This was back in 2014 and to my knowledge hasn't changed.

Can we code vascular dementia on its own or does it need to coded with a underlying physiological condition?
I don't have the cause of the vascular dementia and under F01 it states to "code first" and vascular dementia does not appear in brackets so it is not part of a pure manifestation/etiology pair.
I am coding hospice and the terminal is the vascular dementia but that is an invalid Terminal DX.
