Skin graft

Hello,

I have a patient being admitted to HH for wound care.. They have squamous cell carcinoma of the lower leg, they done a skin graft with patient being the donor. We are doing wound care to both sites. Would I code this as
C44.729, Z4801, Z945, and Z5210?

    I believe the last code should be Z52.11 along with Z48.3 as the primary
    code.
