Pressure Ulcer on a recertification

We are recertifying a client who had a stage 2 pressure ulcer which has turned into a stage 3 pressure ulcer. Do we remove the stage 2 code and add a stage 3 pressure ulcer code? Or do we code both stages even though it is the same ulcer, just worsened.

    Take the Stage 2 code out and use the Stage 3 code. You can increase the stage of a wound but never down stage a wound.

    It is no longer a stage 2 Ulcer it has evolved into a Stage 3 Pressure Ulcer
