Adding wound diagnosis without order
I am new to Home Health coding and I am wondering if a wound is documented, can this be coded without physician signing? We have a sheet that says if we don't hear from you we are adding diagnosis but I am not sure this is ok to do. Any help is appreciated. Thank you.
