htn, heart, kidney, dm2

Forgive me if this has been discussed before, but I need advice. Patient has htn, chronic diastolic heart failure, stage 3 ckd, and dm2.
Are these correct? I13.0
I50.32
N18.3
E11.9 (there is no medical documentation of dm2 with ckd stage 3)

I believe that we are to assume a relationship between htn, heart failure, and ckd, if physician doesn't state another reason.
If the physician doesn't list or state diabetic kidney or diabetes with kidney failure, do I need to assume dm and kidney failure are always related? I don't think that, but need to know for certain.

    I think I was confused thinking that the physician's wording had to say "with". Thanks for clarifying that "with" is assumed in a patient that has those diagnoses.
