PT/INR checks

in Home Care Coding General
Patient's primary reason for home health is PT/INR checks for coumadin use, for past DVT's. Would I code Z51.81 (encounter for therapeutic drug level monitoring as primary)? Since patient does not have an active DVT at the moment.

    So with private insurance you would use Z51.81?

    You cannot just admit for lab in general, however if the PT/INR levels are not stable and the MD is making frequent adjustment. This patient may require medication management / dose changes and instruction on the proper diet ... then I would use

    Z86.718 history of DVT
    Z51.81 Monitoring Drug use
    Z79.01 anticoagulant use
