Coding moisture related skin breakdown

Needing advice on what to code for "moisture related skin breakdown". Nurse found what she initially called "Incontinence wound" on patients hip. When I asked for MD confirmed diagnosis we received "okay to use calmoseptine on moisture related skin breakdown". This may be description WOCN using to describe such an area of breakdown but I do not know "type" of wound to code. Any advice is appreciated.

    If from sheering and moving in bed it is a stage 2 pressure ulcer at least depending on the depth

  • Moisture-related skin breakdown is a dermatitis, not a pressure ulcer.
    The constant moisture irritates the skin like a diaper rash which is also a dermatitis.

    Yes---I just had the same dx. last week and used the dermatitis code as well.
