Do you send out a POC for a ROC when there is significant changes in the POC.

    If there is a significant change in condition a new assessment(a recert OASIS with option 5 'other follow up' selected on M0100) must be done and any new orders with a T.O. - you don't re-do the 485 until it's time to recertify for another 60 day period.

    No.
    ROC should have a verbal order which updates the POC for the certified (or recertified) period.

    I should add that therapy does a POT though (HH-10a)

    We send out ROC 'orders' which acts as an addendum to the 485. The next POC (485) would be sent out at recert.
