right brca with mastectomy with tram flap recon

Would this be a case where Z42.1 used rather than the z48.3?

    I wouldn't think so. If the agency is providing postop care following the mastectomy d/t breast cancer with simultaneous reconstruction I would code Z48.3 and then the BRCA.
