subtherapeutic INR on coumadin

I come up with r79.1 But there is a coding tip that says it is not for those on blood thinners and to use z51.81 and z79.01 instead. I can't find anything that says it is an adverse effect of or that is wasn't properly administered. This is what she was treated for in the hospital so I am unsure of what to use.

  • Take a look at D68.32 drug induce hemorhagic disorder
    use additional code for adverse effect if applicable (T45.515 or T45.525)
  • I thought about that too but the chart didn't mention any type of disorder so I wasn't sure. Thanks for the input, I appreciate it!

  • I did not because there was no mention of underdosing but I will ask the nurse if she has any more info on how it was given. Thanks!
  • Subtherapeutic INR means the Coumadin dose was too low and has to be increased to get the INR back to therapeutic range 2-3.
