If you don't have a Care Plan

Per Medicare Policies

CNA's work under the supervision of an RN; if a CNA does not have a plan of care then the CNA cannot enter the premises to provide care until the order for HHA and care plan are entered in the system; and the correct discipline is selected.


CNA's must perform 70% of their visit providing hands on care.

Charting must be completed within 48 hours of a visit this is a CMS requirement for all staff (excludes SOC).

This validates your visit and permits charting to support the plan of care; and if CNA's are not providing hands on care for a patient, then this is Medicare Fraud. CNA's or Nursing cannot chart something you do not do; this is Falsification of a medical record.

This places the Agency liable and the CNA/RN as well and can result in the loss of your certification/licensure and sanctions placed on the agency.

CNA'S
If you enter the home and you don't have a plan of care then you cannot do anything for the patient and you should notify the RN. Call from the residence so the patient sees that you are attempting to resolve this. Leave a message for the RN if they do not answer and of course you should also follow up with an email to Kim Holterman, the RN Case Manager, and the office; you can send it to me if you wish I can notify the RN also. Just add each of us to the same email in the communicate app. You can also use another email if you please. If using another email to comply with HIPPA just enter the pt ID.
