What do we do with HTN, CKD or ESRD, and DM all in the referral?

Should we continue to assume the association of CKD with the HTN? And code the E11.9?

Do we use E11.22 if the physician does not note the causal relationship between the two(DM and renal)? I know we assume the relationship between the HTN and CKD.
