Pressure Ulcer Not Visualized by RN on Admit
A patient has 2 known PU documented from MD as Stage 3. The SN does not visualize the PU on admission due to the wound care performed before she arrived. She marked the PU on the Oasis, I was thinking that we would not be able to capture them on the Oasis since she did not see the wounds? Would we need to mark unstageable due to non removable dressing? She also did not make another visit within 5 days another RN did. The other question I have is with regards to the Coding I was thinking that we can code what MD documented the stages are, but just cannot capture on Oasis.
