Porta cath care code

I have a patient in home health who has breast cancer, who has had a bilateral mastectomy and is still on chemo. Her referral states 'porta cath flush monthly'. I believe this is the same as surgical wound care but I'm not sure. Since she's had the mastectomy, I don't have the site where the breast cancer originated so would I code primary as Z48.01, surgical wound care, C50.91 breast cancer unspecified site and absence of breast Z90.13?

  • I would code attention to VAD if that's what you are seeing the pt for. Then the others.
  • The "porta-Cath" is a surgical wound for OASIS purposes.
    Port-a-cath care should be coded to Z45.2 Encounter for adjustment and management of vascular access device
