Post-op complication

Looking for help coding this patient. This is what the doctor's note says: Pt with prostate CA s/p radical prostatectomy with intraoperative anterior extraperitoneal rectal wall injury. Open rectal pubic radical prostatectomy, pelvic lymphadenectomy, repair of rectal injury.
This injury required colorectal surgery to perform primary full-thickness repair of anterior extraperitoneal rectal injury. exploratory laparotomy, omental interposition flap between extra-peritoneal bladder and rectum, diverting loop sigmoid colostomy and drain placement.

Wondering about whether you could use AC surgery of neoplasm since the prostate wasn't complicated, then use the complication of post-op surgery of rectum??
